Assessment: For each new client, Care NOLA conducts a comprehensive assessment of needs, identifying immediate concerns. Focusing on client resources, urgency and overall goals, solution-based strategies are developed.

Care Plan: Care NOLA provides regular client visits to address agreed upon care management activities.

Implementation: Our belief is that consistent communication ensures a great majority of issues are addressed speedily and appropriately. Care NOLA will create and maintain a dedicated communication link across all involved parties, including health care providers, to lower family stress, avoid crises and reduce expenses.
When we suddenly needed to assess the hospital care, nursing care, and ultimately hospice care options for my ill Mother, we reached out to Rachel for advice. She brought a wealth of understanding and compassion to the situation and counseled us in reviewing our options and making appropriate arrangements.
